Patient Instructions
- Please call (815) 459-7400 to schedule an appointment. Please be prepared to provide the following information:
- All medical conditions
- Name and dosages of medications
- Insurance information
- On the day of your appointment, please bring:
- This sheet.
- Any X-rays that your dentist has given you.
- Any medical and Dental insurance cards you may have.
- A list of all medications with dosages that you are taking.
- The name and contact information for all of the doctors or specialists you are currently seeing.
- If you are going to have sedation or general anesthesia (going to sleep):
- Do not eat or drink anything for at least 8 hours before your appointment.
- An adult must accompany and stay with you at home after your visit.

Minors (under the age of 18) must be accompanied by a parent or legal guardian.
- Please wear loose clothing, preferably with short sleeves.
- If you are unable to keep your appointment, please call (815) 459-7400 at least 24 hours before your appointment to reschedule.

Our office is conveniently located at 850 Munshaw Lane in Crystal Lakes, off Rakow Road, one block west of the intersection of Route 31 & Rakow Road, next to the Barn Nursery.

- Monday, Tuesday & Thursday 8:00 AM - 4:30 PM
- Wednesday & Friday 8:00 AM - 3:00 PM
We have a 24-hour answering service for after-hours calls.
